Community Living Supports

Description

Job Summary

The Community Living Supports (CLS) position providers a services to facilitate independence and promote integration into the community for a participant residing in his or her home or in his or her family home served under the Michelle P. Medicaid waivers.

Responsibilities

>Provide supports and assistance related to chosen outcomes on client's PSCP

>Ensure supports are in compliance with Medicaid regulations

>Collaborate effectively with each participants case manager and other supports providers/agencies to ensure that services are person centered

> Advocate for and be a resource to participates and their families. Allow self-determination and input from families to guide services.

>Practice safety and report all workplace accidents according to KPI Therapy polices

>Ensure incident reports and follow up incident reports are filled and sent to case managers in a timely manner for all incident concerning participants

>Perform all other related duties, as required by the SCL and Michelle P. Medicaid waiver programs

>Maintain cordial working relationships with outside agencies providing waiver services/case managements

>Ensure that billing issues are addressed and help the accounting department troubleshoot any Medical payment errors.

Qualifications

18 years of age

Pass background check

Pass drug test

CPR/First Aid needed

Proof of car insurance and registration
