Patient Engagement Specialist (Remote)

Location: Remote 100%

Position Type: Full Time (40 hours per week)

Compensation: Hourly Pay + Bonus Opportunities

Shift: 8am - 5pm or 10am - 7pm (EST or CST)

Travel: None


We are hiring a friendly, motivated Patient Engagement Specialist to join our client’s patient engagement team and help enroll patients into a life-changing Chronic Care Management (CCM) program, all while building meaningful relationships and having fun along the way! In this role, you’ll be the welcoming voice of our organization, reaching out to patients with long-term health needs (such as diabetes, respiratory conditions, or COPD) and helping them access the ongoing care they deserve.


You’ll connect with patients over the phone, educate them on how chronic care management works, and enroll them into the program that improves their health, reduces hospital visits, and provides accountability between doctor appointments. Key Responsibilities:

  • Make high-volume outbound calls to patients to introduce chronic care management and remote patient monitoring services, determining patient eligibility based on insurance coverage and program requirements.
  • Educate and sell patients on the benefits of CCM and how participation can support their long-term health goals
  • Use provided scripts and patient information to tailor conversations, address concerns, and build rapport with empathy and professionalism
  • Identify interest and eligibility through effective questioning and active listening
  • Overcome objections by addressing patient concerns with compassion and accurate information
  • Obtain verbal agreement for patient enrollment into the care management program
  • Accurately document all outreach, responses, and call outcomes in the CRM or care coordination system
  • Collaborate with clinical care teams to ensure smooth handoffs and alignment on next steps
  • Track engagement and follow-up with non-responsive patients via multiple outreach attempts
  • Stay informed on RPM program changes, Medicare eligibility requirements, and service updates
  • Use digital tools such as VoIP systems, EMR, dashboards, and autodialing and call tracking software to manage daily workflow efficiently
  • Apply emotional intelligence and conversational agility to personalize outreach and improve patient conversion rates
  • Continuously meet or exceed daily and weekly outreach goals and performance benchmarks
  • Adapt to new technology platforms and scripts as services and systems evolve
  • Meet or exceed individual outreach quotas and contribute to team enrollment goals

Qualifications:

  • Minimum 1 year of experience in healthcare, customer service, or sales environment, such as outbound telemarketing, inside medical sales, or healthcare call center roles
  • High school diploma or GED required; college degree or healthcare/customer service certifications a plus
  • Familiarity with healthcare terms and services preferred, especially related to chronic care management, Medicare, or remote patient monitoring
  • Strong verbal communication and sales skills with the ability to clearly explain medical services and build trust over the phone
  • Prior experience in medical, mental health, or chronic care outreach is preferred
  • Proven experience in outbound telemarketing, patient outreach, or inside sales - healthcare or senior patient population experience strongly preferred
  • Strong verbal and written communication skills with a confident, empathetic approach
  • Proven ability to build rapport quickly and professionally handle objections
  • Comfortable working independently and in a fast-paced, metrics-driven environment
  • Familiarity with CRM platforms, VoIP systems, and basic healthcare technology; EHR experience is a plus
  • Understanding of HIPAA and commitment to maintaining patient confidentiality
  • Compassionate and patient-centered mindset
  • Empathetic and patient-focused approach, especially when speaking with seniors or individuals managing chronic conditions
  • Organized and detail-oriented with strong documentation and data entry skills
  • Adaptability to changing scripts, service updates, and outreach strategies
