Person Centered Coach

Description

Job Summary

Person Centered Coach- (PCC) providers community -based support services. Adult with developmental disabilities are the primary population by Person Centered Coach. The Person Centered Coach, in conjunction with the Behavior Specialist and Case manager, ensure the proper implementation of person centered support plans.

Responsibilities

>Ensure that all behavior support plans are implemented, as written

>Implement, train and monitor behavior support plan using positive behavior supports

>Assists a participant and participant's person- centered team in implementing and monitoring the effectiveness of participants person- centered service plan

>Models person-centered thinking

>Is responsible for training participant, family, guardian, natural and unpaid supports, and other members of the person-centered team when barriers challenge the success of the participant in achieving his or her goals.

>Provider services in a timely manner, within 15 days of service authorization

>Complete service requirements as outlined in SCL, MPW regulations

>Complete quarterly reports which include both behavior data and verification of time spent with participant

> Attend POC meetings, quarterly staff meetings and psychiatric appointments when needed

> Maintain constant communication with Behavior Specialist, Case Manager and other members of the person-centered team

>Be available or have established an available back up, by phone, 24/7

>Submit Critical Incident Reports to DSBS/DDID and complete necessary follow-up documentation follow all procedures as required by DDID

>Follow as necessary documentation requirements as directed

>Performs other duties as needed

Qualifications

> A high school diploma or GED; and either (2) two years of experience in the field of intellectual of developmental disabilities; or completed twelve (12) hours of college coursework in a human services field

>Continuing education, if applicable

>Minimum two years of experience in the field of development disabilities

>12 hours of college credits in a human services field if 2 years of experience is not met
